DEADLINES FOR FALL 2012, & SPRING 2013 STUDENT TEACHERS
Fall 2012 Student Teachers will need to provide a TB Test, ACT 34 Criminal Record Check, ACT 151 Child Abuse Clearance, PA FBI Clearance, and Professional Liability Insurance dated no earlier than February 1, 2012 and no later than June 30, 2012.
Spring 2013 Student Teachers will need to provide a TB Test, ACT 34 Criminal Record Check, ACT 151 Child Abuse Clearance, PA FBI Clearance, and Professional Liability Insurance dated no earlier than June 15, 2012 and no later than September 30, 2012.
1. To renew the TB Tine/Mantoux for Student Teaching:
Visit the campus clinic for a TB Tine test. You will be provided with two parts of a three part form, one for your records and one to bring to the Field Experience Office.
OR
Visit your personal physician for a TB Tine test. Have your physician provide a prescription sheet/ letterhead (for the Field Experience Office) including the following:
Patient name, social security number, and date of birth
Office name and location
Results (negative/positive) of TB Tine/Mantoux test
Doctor’s signature
2. Criminal Record Check (ACT 34): Pennsylvania Access to Criminal History for use with a credit card or Act 34 Clearance (slow!)
3. Child Abuse Clearance (ACT 151): Act 151 Clearance
4. PA FBI Clearance: Cogent Systems
5. Professional Liability Insurance:
Pennsylvania State Education Association (SPSEA)
Council for Exceptional Children (CEC)
The National Association for Music Education (MENC)
Be sure to print out proof of 2012-13 membership (for Fall 2012 or Spring 2013 Student Teaching) to provide to the office.
